GAP provides emergency assistance to those experiencing homelessness and low-income households. Assistance includes clothing and footwear for all ages, food boxes every 30 days, prescription vouchers for medications, personal hygiene items, and local bus passes. Some our services are determined by resource availability and may not be available year-round.
Case management is available to those we serve. Case management is a collaborative process used in our program to assess, plan, implement, coordinate, monitor, and evaluate the services needed by individuals and families to achieve specific goals. It is designed to help clients navigate complex systems, overcome barriers, and access the resources and support they need.
If you are in need of assistance, we strive to provide resources quickly and efficiently. The below information is necessary to bring with you when you visit our offices. We need all documentation each time aid is requested. Please bring all of the following that applies to you:
01Proof of All Expenses
Must be in the person requesting assistance or the spouse of the person requesting assistance’s name.
Child Support
Cell Phone
Medical
Insurance
Rent or Mortgage
Utility Bills
Original AND cut-off notice if applying for utility assistance
Any other fixed monthly bill
02Proof of Household Gross Monthly Income
For all household members over the age of 18.
Gross monthly income is the amount of income before any withholdings for taxes, insurance, retirement contributions, etc.
If paid weekly, provide at least 8 pay stubs.
If paid bi-weekly, provide at least 4 pay stubs.
For SSI/SSD, provide current Social Security Award.
